Can someone help me find a plugin that's similar to the AwakenSMP plugin?

I really need a plugin where, after being killed by another player a configurable number of times, the player drops their head and gets banned from the server. They can only be revived if someone places their original head. Unlike the original Awaken SMP plugin, you can't craft or rename a head to revive someone—only the real, original head of that specific player works. If the head is destroyed, despawned, or lost, the player remains permanently banned.
